Help! severe issues and not sure what to do?

I have a friend who has not done anything with their teeth for years. This person does not brush, period. This is painful to look at and i cant imagine being in this situation. there is apparently no discomfort with this situation yet, but im wondering how long until this will require the extraction of multiple teeth? if regular dental care was started at this point, is it too late? how can i tell this person they need to fix this problem?

My uncle never brushes his teeth. They had to cut his gums and scrape tartar off of his roots . . . twice! He didn't learn the first time. He still hasn't learned. But I don't know if a procedure like that would be able to save your friend from having teeth pulled. But I don't think anyone here can really answer the first two of your questions because none of us can look at your friend's teeth and see how rotten, cavity-filled, and tartar-covered they are. In terms of the last question, I'd just be honest with your friend. Don't say "Your breath stinks and your teeth look gross and I can't stand being around you" or anything like that. But say you're concerned about him/her having to have all of his/her teeth extracted.
