I see movies taking place centuries before, and everyone has good, healthy teeth. I can't help but imagine that everyone from that time had all of their teeth rotten/missing. Was this the case?

My best guess is that it would be somewhere inbetween. They didn't consume nearly as much sugar then, which is something to take into consideration. Also consider that fluoride is very toxic and not necessarily proven to truly strengthen teeth. Victor Frankl wrote Man's Search For Meaning about surviving the holocaust. He was a doctor who noted that though the prisoners had, obviously, no dental care, they had healthier gums and teeth than usual. With that said, I don't think everyone's teeth would have necessarily been rotten, but would they look like they just peeled off their last Crest Whitestrip? Yeah, nooooo. And I laugh about things like that when I'm watching movies too. Silly Hollywood.
