In PA if you decline Dental care employer offered benefit must you sign a declination form?

If you choose not to take dental coverage offered by your employer must you sign a form declining and does the employer have to have everyone enrolled or not sign one acknowledging it was offered.

I think that's true in most (if not all) states now, for all forms of insurance. That just proves that they offered you the insurance, so you can't later claim discrimination. It's not something to worry about. Just sign it if you don't want the insurance.
